Two different things
This website and your medical record are separate.
This website does not collect health information. There is no form on it that asks about your health, and nothing you read here is recorded against your name.
Booking, enrolment and the patient portal are handled by our clinical systems, which are separate from this website and governed by Ontario's Personal Health Information Protection Act, PHIPA.
What this website does
- Remembers whether you chose the light or dark theme, on your own device
- Links out to booking, enrolment and the patient portal, which are run on separate systems
- Embeds a map from Google so you can find the unit
- Does not ask for, store or transmit any health information
What the clinic holds
- Your health card number and contact details
- Your medical history, visit notes, prescriptions and test results
- Records requested from a previous practice, with your consent
- Referrals and reports shared with specialists involved in your care

See your record
You can ask for a copy of the information the clinic holds about you.
Correct a mistake
If something in your record is wrong, you can ask us to correct it.
Know who has seen it
You can ask who your information has been shared with, and why.
Withdraw consent
You can limit what is shared, though it may affect the care we can provide.
Transfer your record
If you move practice, we send your record to your new physician on request.
Raise a concern
Tell us first, and if we cannot resolve it you can contact the Information and Privacy Commissioner of Ontario.

Requests about your own record are handled by the clinic at 1005 Paisley Road. An interpreter can be arranged for the conversation at no cost to you.

New patients are welcome at 1005 Paisley Road. Bring your health card, and tell us if you would like an interpreter.
